Case report: Novel technique of flexor pollicis longus tendon transposition and reconstruction using flexor carpi radialis graft

Abstract: Introduction: Flexor pollicis longus (FPL) rupture is a debilitating condition. A graft is necessary when primary repair is not possible. There are different graft options available for FPL reconstruction. This study aims to evaluate different graft options available and to report on the use of flexor carpi radialis (FCR) tendon graft in a novel technique for FPL reconstruction. Case presentation: A 41-year-old male with loss of thumb interphalangeal joint flexion secondary to chronic FPL tendon rupture after distal radius plate fixation 10 months prior to presentation. In context of overtight primary repair and absent palmaris tendon, reconstruction was achieved with FPL transposition and FCR splitting and grafting restoring thumb interphalangeal joint flexion. Postoperatively, the wrist was maintained in flexion with a dorsal splint. Conclusion: This case presents FCR graft in a novel technique and as an effective option when palmaris graft is not available.
