Zone V volar forearm wound with unicortical radius fracture with a missed tendon injury: A case report

Abstract: Flexor tendon injuries are a common sighting in the emergency orthopaedics department in any institution. Though flexor tendon injuries at the level of forearm are not infrequent, most of the literature has focussed on flexor tendon injuries in the hand especially on zone II. It is often believed that tips and tricks utilized in this zone can be extrapolated to zone III to V injuries as well. Tendons, nerves, and blood vessels in zone V are located in close proximity with high risk of combined injuries to multiple structures from a seemingly innocuous small looking wound. As such, forearm injuries with sharp objects resulting in bone deep injuries/ fracture is almost always associated with some structure damage to any myotendinous/ neurovascular structure. We present a rare case of wound over flexor aspect of forearm caused by a sharp object resulting in a unicortical fracture of the radius without any apparent neurovascular / tendon injury.
