Abstract: Simultaneous bilateral patellar tendon rupture (SBPTR) is a rare condition and has been frequently linked to systemic diseases, including systemic lupus erythematosus (SLE). We present the case of a 40-year-old Saudi female with SLE who developed SBPTR following a simple fall. This report outlines the diagnosis and management and includes a review of the literature on SBPTR in patients with systemic conditions. Early diagnosis and prompt surgical intervention are crucial for favorable outcomes, especially in those with underlying systemic diseases.
